New Medicaid “work requirements” are coming, caregivers are exempt, but you may have to prove it

If you’re a family caregiver who relies on Medicaid for your own health coverage, here’s something to keep on your radar, not to panic about, but to be ready for.

What’s changing

The federal law known as the One Big Beautiful Bill Act (OBBBA) introduces new “community engagement requirements”, often called work requirements, for some adult Medicaid beneficiaries. Under them, affected people will need to show at least 80 hours a month of work, training, or other qualifying activities to keep coverage.[1]

The important part for caregivers

Family caregivers are exempt from the requirement, specifically those responsible for children age 13 and under, or for a disabled individual of any age.[2] That’s genuinely good news. But there’s a catch worth underlining:

  • The exemption is not automatic. States must proactively build the systems that recognize caregivers as exempt, and in many cases it will fall to you to prove you qualify.[1]
  • The timeline gives some breathing room. States have until January 1, 2027, to adopt the requirements, and those demonstrating progress may seek a one-time extension of up to two years.[2]

What to do now

Nothing urgent today, but watch your mail. The single biggest risk in coverage changes like this is a missed renewal notice or a form that doesn’t get returned. Make sure your address is current with your state Medicaid office, open everything they send, and if you get anything mentioning work or community-engagement requirements, look for how to claim the caregiver exemption. AARP and your state’s Medicaid office are tracking implementation closely.[1]
